Immunization and Vaccination Requirements
To protect the health and well-being of the campus community, students are required to have vaccination and immunization records on file with Health Services.
NOTE: Students who are taking fully online or remote classes and do not utilize in-person services, do not have to comply with the requirements listed below.
Requirements
COVID-19 Vaccination
SUNY no longer requires students to be vaccinated for COVID-19, however, all members of the campus community are strongly encouraged to do so, and to stay up to date on boosters as recommended by the CDC in order to protect themselves and others. Those who have received their vaccines and/or boosters are asked to continue to submit their updated vaccination records using the appropriate Health Services form, in order to ensure accurate recordkeeping.
NOTE: All students enrolled in an applied learning experience, program or course requiring a clinical or internship will still be required to comply with their host site policies, including any vaccination or testing protocols. This applies to students in any academic program and all levels of clinical and allied health training.
Exemptions
Exemptions to the above requirements may be requested based on either religious conviction or health reasons. To complete the exemption process, complete the appropriate Health Services form and return it along with any required documentation by the deadline listed.
NOTE: Health Science students are not eligible to receive an exemption. Students enrolled in Health Science programs that have a hospital clinical component, must have the appropriate vaccinations and/or immunizations to attend clinical rotations and therefore complete these programs.
Process for Obtaining Records
- Once your application has been processed, Health Services will begin the process of collecting the required vaccination and immunization records on file with the New York State Immunization Information System (NYSIIS).
- The information collected through NYSIIS will be recorded and marked on the student's My Enrollment Checklist in WIReD.
- If the vaccination and/or immunization records are not available on NYSIIS, the student will need to provide the appropriate documentation in order to take any in-person classes or utilize in-person services.
Submitting Documentation
All immunization and vaccination records submitted to Health Services must have an original signature or a practice stamp. Photocopied signatures and stamps are not accepted.
